· Fill in the HISTORY SHEET / FOLLOW-UP SHEET thoroughly, send to drsureshsachdeva@gmail.com or send at the address mentioned on the PATIENT HISTORY SHEET through COURIER/POST.

· If any further information is needed, you will be contacted through E-mail (given by you).

· After analyzing and processing the information provided, line of treatment will be decided according to homeopathic principles.

· Medicines will be sent at your specified destination along with detailed instructions.

· During treatment, changes observed can be reported periodically to drsureshsachdeva@gmail.com using FOLLOW-UP SHEET, so as to help you better. Read the Guidelines and Dietary Precautions thoroughly.

· Medicines are made up of "cane sugar globules" medicated with homeopathic dilutions and are packed in unbreakable plastic vials, securely placed in a box.
