  • Fill in the HISTORY SHEET / FOLLOW-UP SHEET thoroughly, send to drsureshsachdeva@gmail.com or send at the address mentioned on the PATIENT HISTORY SHEET  through COURIER/POST.

  • If any further information is needed, you will be contacted through E-mail (given by you).

  • After analyzing and processing the information provided, line of treatment will be decided according to homeopathic principles.

  • Prescription will be sent by E-mail along with detailed instructions.

  • During treatment, changes observed can be reported periodically to drsureshsachdeva@gmail.com using FOLLOW-UP SHEET, so as to help you better. Read the Guidelines and Dietary Precautions thoroughly.
